JOB SUMMARY

Under the direct supervision of the CalAIM PM, the APM will be responsible for assisting with the oversight and development of the Champions CalAIM ECM and CS, PSH, and Street Outreach Programs in Kings and Tulare Counties. This includes collaboration with several local entities, including County Behavioral Health, County Health and Human Services Agency, Public Health, Sheriff’s Office, County Probation, Adventist Health, CalViva/HealthNet, Anthem Blue Cross, Kaiser, Kings Tulare Homeless Alliance (KTHA), and other community-based organizations to provide integrated services and care management for clientele. The APM will ensure effective day-to-day operations within their assigned programs and provide appropriate training and modeling for staff, including Lead Care Managers, Clinical Consultants, Housing Navigators, Outreach Specialists, and Office Assistants.

The APM will assist in conducting weekly caseload audits for program clientele and ensure appropriate and ethical documentation is maintained for all cases within the electronic health record system. The APM will ensure compliance with state and federal laws and regulations, in addition to the Policies & Procedures set-forth by Champions, DHCS, and managed care plans.

RESPONSIBILITIES AND DUTIES

  • Assist in overseeing and coordinating the allocation of designated resources, overview the record-keeping and reporting functions, and ensure compliance with stated mission, goals, regulations, and guidelines.

  • Evaluate program operations and activities in terms of overall effectiveness and compliance; implement improvements and modifications as necessary; and adjust overall goals and objectives in response to program directives and/or client needs.

  • Assist in developing, coordinating, and participating in collaborative activities, programs, and projects to maximize available resources and responsiveness to clients; provide necessary and requested resources; act as team member/ leader for assigned projects/opportunities.

  • Supervise assigned staff, including training, scheduling, completing performance reviews, and issuing disciplinary actions as needed.

  • Assist in providing supervision for all other ECM, CS, and PSH staff. Identify staff training needs and ensure the development and implementation of programs that address needs, participate in delivering training sessions to meet the needs of staff related to clinical/program needs.

  • Assist in development, implementation, and management systems and processes to maximize the effectiveness and responsiveness of the ECM, CS, and PSH Programs; act as professional and managerial resource to program staff, project teams, and clients in a positive and supportive manner.

  • Collaborate with other work units, agencies, and ECM, CS, and PSH partners to secure advice, resources, and technical services necessary to achieve ECM, CS, and PSH goals, objectives, and directives in accordance with mutually agreed upon time frames and in compliance with pertinent guidelines and regulations.

  • Participate in staff meetings, in order to address client needs, and program issues, and assist in the integration of services for clientele. 

  • Assist and help develop, implement, and manage implementation of Kings County Street Outreach Plan, including the scheduling of street outreach teams with mobile office/trailer and team of clinicians.

  • Assist and help develop, implement, and manage implementation of Kings County Rapid Re-Housing Program, Short-Term Post-Hospitalization Housing, Recuperative Care, and Sobering Center programs.

  • Assist in the preparation of capacity building grants to expand the scope of CalAIM services offered geographically and in types of services offered (i.e. sobering center, transitional housing, etc.)

  • Travel between offices in Kings and Tulare Counties to oversee operations and staff.

  • Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.

  • Remains culturally sensitive and respectful in all client and staff interactions.

  • Adheres to all client confidentiality requirements & standards.

  • Performs other duties as assigned.   

QUALIFICATIONS AND SKILLS

  • Experience: One year of clinical and/or administrative experience in human services, behavioral health, substance use disorder, or related programs, including at least one year of supervisory responsibility; experience working with individuals with a history of mental illness and substance abuse; knowledge of co-occurring disorders and evidence-based treatment approaches. Proficient in the area of program development and evaluation. Working knowledge state and federal regulations relating to mental health services and substance abuse treatment; including HIPAA, 42 CFR Part 2, and Medi-Cal insurance. Must possess knowledge and skills related to social service case management, supervising program service delivery, and outcome measurements.

  • Education: 1) Bachelor's degree (or higher) in social work, psychology, human services, or related field and 1 year of supervisory experience; 2) High school diploma and 2 years of experience managing social service programs.

  • License or Certificate: Valid California Class C Driver’s License and clean driving record required (verified by DMV at hire and periodically). First Aid and CPR certifications obtained within 90 days of employment and maintain current thereafter.

  • Other: Criminal background clearance required.

PHYSICAL SKILLS AND ABILITIES

  • Ability to perform the essential job functions consistently safely and successfully with the ADA, FMLA, and other federal, state, and local standards, including meeting qualitative and/or quantitative productivity standards.

  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards.

  • Ability to participate in street outreach and homeless encampments year-round on undeveloped land without accessibility improvements.

  • Prolonged periods of sitting at a desk and working on a computer.

  • Must be able to lift and carry up to 25 lbs.

  • Must be able to talk, listen, and speak clearly on the telephone.

WORKING CONDITIONS

  • Interfaces with all levels of management, staff, clients, residents, and families including community partners.  Services may involve travel to offsite locations. 

  • Hours:  Work hours can vary and may require some on-call scheduling including weekends, evenings, and holidays per program requirements.
