CalAIM identifies and manages comprehensive needs through whole person care approaches and social drivers of health. We aim to improve quality outcomes, reduce health disparities and transform the delivery system through value-based initiatives, modernization and payment reform. CalAIM makes Medi-Cal a more consistent and seamless system for enrollees to navigate be reducing complexity and increasing flexibility. 

We offer two main programs Enhanced Care Management and Community Supports.

The goal is to identify health barriers and work to reduce health difficulties. We will identify health needs; create goals and provide ongoing support in effort to reach goals; link to supportive services; link to local community resources for utilities, food, and clothing.

Enhanced Care Management

Community Supports

We provide Housing Navigation and Housing Tenancy and Sustaining Services. This can include assistance to secure temporary transitional and/or permanent placement, assist with completing rental applications, enroll into programs within the community.

ELEGIBILITY CRITERIA

General Requirements

  • Medi-Cal insurance must be active in Kings County - Anthem or CalVIVA

And meet one target population:

  • Substance use disorder

  • Severe mental health illness

  • Individuals and families experience homelessness

  • High utilization of the following:

    1. 5 or more ER visits in a 12 month period

    2. Overdoes

    3. Suicide Attempts

    4. Urgent care/inpatient stays

    5. Consistent contact with law enforcement
