POSITION SUMMARY
Under the direct supervision of the Program Manager, the Case Manager will provide weekly group sessions adhering to program Curriculums: (1) Nurturing Parenting, (2) Celebrating Families, (3) ¡Celebrando Familias! (4) Why Try, and (5) Supporting Father's Involvement. The Case Manager will deliver services in a group office setting and provide the participants with skills, knowledge, resources, and support through psychoeducation, case management, to help equip the participants of the programs to make informed and prosocial decisions. Ensure compliance with state and federal laws and regulations, in addition to the policies and procedures set-forth by Champions and maintain appropriate documentation within the electronic client records; which includes the use of an EHR system. Furthermore, work collaboratively with all divisions and provide integrative treatment services and ensure proper and ethical documentation for services and comprehensive service delivery to clientele.
RESPONSIBILITIES AND DUTIES
Complies with all program policies and procedures; including State/Federal or Certification/Licensure requirements.
Maintains a caseload of clientele and provide active and comprehensive case management services. Ensuring all area needs are attended to (mental, substance abuse, social, physical, and family).
Ensures appropriate and comprehensive documentation within an EHR system.
Monitoring the health and welfare of the consumer and the quality of services provided to the consumer through contact while enrolled.
Participates in staff meetings, in order to address client needs, program issues, and assist in the integration of services for clientele.
Facilitate Psycho-educational groups and classes, as deemed appropriate by management.
Cook for the families following all food handling procedures.
Enter new clients onto sign in sheet, client roster, and enrollment spreadsheet.
Enter Adult Adolescent Parenting Inventory scores online.
Provides integration of all service divisions of the program, specifically acting as a bridge between disciplines such as mental health and probation.
Develops and maintains cooperative work relationships with community agencies and partners, including client connections to outside agencies (i.e., court hearings, open CPS cases and requirements, etc.)
Determines appropriate services available through the Parenting/Youth program and/or referral to other community agencies.
Complete intakes and eligibility for potential clientele and verify completeness and accuracy of documents.
Serve as the main point of contact for all clients and relay client needs to the appropriate channels.
Participate in applicable meetings, training, conferences, etc. as required both internally and externally.
Participate in outreaching within the community.
Remains culturally sensitive and respectful in all client and staff interactions.
Adheres to all client confidentiality requirements and standards, including updates of the shared Release of Information (ROI) to ensure that information is current and accurate.
Comfortable performing various projects in conjunction with day-to-day activities.
Ability to get along with diverse personalities; understanding, mature, and flexible.
Possess strong judgment, problem-solving and reasoning abilities.
Perform other duties as assigned.
QUALIFICATIONS AND SKILLS
BA in social work, psychology, or related field + 2 years’ mental health experience; or AA in social work, psychology, or related field + 3 years’ mental health experience and/or other relevant degree(s) and related experience may be substituted and/or required and working towards a bachelor’s degree in the health or human services or related field preferred, and/or at least one-year background and experience working with at-risk youth.
Valid California driver’s license and insurable driving record. (If the position requires driving, valid driver’s license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment.)

Must be CPR and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment.
Excellent organizational, written and oral communication skills
Substance Abuse counseling/support experience desired.
Experience with culturally diverse population.
Ability to understand and interpret information, follow instructions and manage workflow.
Strong Interpersonal skills; ability to work effectively with people in a constructive manner within a community-based treatment setting.
Basic and routine computer keyboarding, programs and software and advanced office management practices and operating procedures.
Able to multi-task in a fast-paced environment, apply professional judgement in solving difficult issues, and to communicate in a clear, precise and professional manner.
Bilingual (English and Spanish speaking) preferred.
ORGANIZATION RELATIONSHIPS/INTERACTIONS:
Interfaces with all levels of management, staff, clients, residents and families including partner agencies.
PHYSICAL SKILLS AND ABILITIES
Must have the visual, physical and technical ability to independently operate a variety of office equipment including the use of a computer. Will also require bending, stooping, filing, lifting of at least 25 lbs. or more and able to interrupt physical altercations. Ability to verbally communicate well with others.
WORKING CONDITIONS
Services may involve travel to offsite locations for assessments, service provision, and crisis management.

ANTICIPATED HOURS
This position may include working non-traditional business hours, swing or graveyard shifts and/or weekends, all varying shifts.
