ORGANIZATIONAL DESCRIPTION

Champions is a nonprofit organization that has served Kings County and the surrounding areas since 2000. Initially specializing in substance abuse treatment and recovery, Champions has grown to become one of the main providers for social and community-based programs within Kings County. Champions programs serve women, women with children, and men as well as those involved in the criminal justice system, homeless populations, cultural and ethnic minorities, and those with co-occurring disorders.

JOB SUMMARY

Under the direct supervision of Program Manager, the Case Manager will provide case management services for all clientele in a comprehensive manner. Ensure compliance with state and federal laws and regulations, in addition to the Policies & Procedures set-forth by Champions and maintain appropriate documentation within the electronic client records; which includes the use of an EHR system. Furthermore, work collaboratively with all divisions and provide integrative treatment services, as well as individual and Psycho-educational groups and classes as deemed appropriate. Work jointly within the KWPC system of care.

The Case Manager will manage and oversee a variety of intensive case management functions within the Parenting Program, which consists of case management duties. The Case Manager will ensure proper and ethical documentation for services and comprehensive service delivery to clientele.

SUMMARY OF ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Complies with all program policies and procedures; including State/Federal or Certification/Licensure requirements.

  • Maintains a caseload of clientele and provide active and comprehensive case management services. Ensuring all area needs are attended to (mental, substance abuse, social, physical, and family).

  • Ensures appropriate and comprehensive documentation within an EHR system.

  • Work collaboratively with KWPC staff and referring parties and service providers.

  • Participates in staff meetings, in order to address client needs, program issues, and assist in the integration of services for clientele.

  • Facilitate Psycho-educational groups and classes, as deemed appropriate by management.

  • Provides integration of all service divisions of the program, specifically acting as a bridge between disciplines such as mental health and probation.

  • Develops and maintains cooperative work relationships with community agencies and partners, including client connections to outside agencies (i.e., court hearings, open CPS cases and requirements, etc.)

  • Determines appropriate services available through the Parenting program and/or referral to other community agencies.

  • Complete intakes and eligibility for potential clientele and verify completeness and accuracy of documents.

  • Serve as the main point of contact for all clients and relay client needs to the appropriate channels.

  • Participate in applicable meetings, training, conferences, etc. as required both internally and externally.

  • Remains culturally sensitive and respectful in all client and staff interactions.

  • Adheres to all client confidentiality requirements and standards, including updates of the shared Release of Information (ROI) to ensure that information is current and accurate.

  • Comfortable performing various projects in conjunction with day-to-day activities.

  • Ability to get along with diverse personalities; understanding, mature, and flexible.

  • Possess strong judgment, problem-solving and reasoning abilities.

  • Perform other duties as assigned.

MINIMUM REQUIREMENTS

  • BA in social work, psychology, or related field + 2 years’ mental health experience; or AA in social work, psychology, or related field + 3 years’ mental health experience and/or other relevant degree(s) and related experience may be substituted

  • Must have valid California Class Driver’s License; maintain a good safety record (verified by DMV at hire and periodically) and a reliable vehicle; proof of liability insurance if driving personal vehicle.

  • First Aid and CPR certifications obtain within 90 days of employment and maintain current thereafter.

  • Experience must be in the provision of case management services for persons living with mental illness. Substance Abuse counseling/support experience desired.

  • Experience with culturally diverse population.

  • Ability to understand and interpret information, follow instructions and manage workflow.

  • Strong Interpersonal skills; ability to work effectively with people in a constructive manner within a community-based treatment setting.

  • Basic and routine computer keyboarding, programs and software and advanced office management practices and operating procedures.

  • Able to multi-task in a fast-paced environment, apply professional judgement in solving difficult issues, and to communicate in a clear, precise and professional manner.

  • Bilingual (English and Spanish speaking) preferred.

  • Criminal background clearance required.

  • Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.

ORGANIZATION RELATIONSHIPS/INTERACTIONS

Interfaces with all levels of management, staff, clients, residents and families including partner agencies.

PHYSICAL SKILLS AND ABILITIES

Must have the visual, physical and technical ability to independently operate a variety of office equipment including the use of a computer. Will also require bending, stooping, filing, lifting of at least 25 lbs. or more and able to interrupt physical altercations. Ability to verbally communicate well with others.

WORKING CONDITIONS

Services may involve travel to offsite locations for assessments, service provision, and crisis management.
