JOB SUMMARY
Under the direct supervision of the Executive Director, Champions’ Housing Program Manager will be responsible for the oversight and development of Champions’ Housing Navigation and Tenancy Programs, Permanent Supportive Housing (PSH), Transitional Housing, and Street Outreach Programs in Kings and Tulare Counties. This includes collaboration with other Champions programs and local entities, including County Behavioral Health, County Health and Human Services Agency, County Public Health, Sheriff’s Office, County Probation, CalViva/HealthNet, Anthem Blue Cross, Kings Tulare Homeless Alliance (KTHA), US Department of Housing & Urban Development (HUD), and other community-based organizations to help individuals struggling with homelessness to locate, secure, and maintain safe housing to meet their overall health needs. The Housing Program Manager will assure effective day-to-day operations of Champions’ Housing Programs and will supervise all staff in these programs. The Housing Program Manager will provide appropriate training and modeling for staff, including Housing Navigators and others.
The Housing Program Manager will conduct weekly caseload audits for program clientele to ensure appropriate and ethical documentation is maintained for all cases within the electronic health record system. They will oversee, evaluate and/or revise Housing Plans as needed for clients enrolled in housing services. The Housing Program Manager will ensure compliance with state and federal laws and regulations, in addition to the Policies & Procedures set-forth by Champions, Depart of Health Care Services (DHCS), HUD, KTHA, and managed care plans.
The Housing Program Manager will oversee an established CalAIM Housing Navigation, Tenancy, and Deposits Program in Kings County and expand these same services into Tulare County. The Housing Program Manager will also expand Champions’ CalAIM services to provide Short-Term Post-Hospitalization Housing services in Kings and Tulare Counties.
RESPONSIBILITIES AND DUTIES
Oversee and coordinate the allocation of designated resources and ensure compliance with stated mission, goals, regulations, and guidelines.
Develop, implement, and manage systems and processes to maximize the effectiveness and responsiveness of Housing Programs.
Manage Champions Housing Programs usage of electronic health records (EHR) systems utilized and reporting responsibilities to all funding agencies, government agencies, and Champions management team.
Evaluate program operations and activities in terms of over-all effectiveness and compliance; implement improvements and modifications as necessary; and adjust overall goals and objectives in response to program directives and/or client needs.
Manage staff assignments, supervise, and train all Champions Housing staff.
Act as professional and managerial resource to program staff, project teams, and clients in a positive and supportive manner.
Collaborate with other Champions Programs, KTHA, community providers, managed care plans, and county agencies to secure advice, resources, and technical services necessary to achieve Housing Program goals, objectives, and directives in accordance with mutually agreed upon time frames and in compliance with pertinent guidelines and regulations.
Coordinate and participate in collaborative community activities, programs, and projects to maximize available resources and responsiveness to clients; provide necessary and requested resources; act as team member/ leader for assigned projects/opportunities.
Participate in budget preparation and administration of Champions Housing Programs; preparing cost estimates/projections, submitting justifications for requested resources, and monitoring and controlling expenditures.
Perform needs assessments and complete individualized housing plans as necessary.
Organize and lead regular staff meetings in order to address client needs, program issues, and assist in the integration of services for clientele.
Coordinate CalAIM services, usage of the EHR system, monthly billing, community outreach, and usage of shared resources with the Enhanced Care Management (ECM) Program Manager.
Develop, implement, and manage implementation of Kings County Street Outreach Plan, including the scheduling of street outreach teams with mobile office/trailer and team of clinicians.
Oversee the implementation, budget, case management, and reporting of HUD PSH grant and services, including the following:
Completion of eligibility screening, intake documentation, comprehensive assessments, and intervention plans that addresses need for housing, services, crisis intervention, and continuity of care planning and coordination.
Identification of appropriate housing options for clients, such as, emergency housing, transitional housing, subsidized housing Section 8, Shelter Plus Care, permanent supportive housing, affordable and market rate housing, and other housing opportunities.
Assistance to clients in completing housing applications and coordination of client move-ins and exits from all housing units.
Survey of local housing markets and services and advocacy for clients with landlords.
Recruit landlords and establish working relationships for the benefit of clientele.
Assist in the preparation of capacity building grants to expand the scope of CalAIM and HUD services offered geographically and in types of services offered.
Research and evaluation of housing developments for Champions to pursue.
Travel between offices to oversee operations and staff.
Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
Remains culturally sensitive and respectful in all client and staff interactions.
Adheres to all client confidentiality requirements & standards.
Performs other duties as assigned.
QUALIFICATIONS AND SKILLS
Experience: Two years of increasingly responsible clinical and/or administrative experience in human services, behavioral health, substance use disorder, or related programs, including at least two years of supervisory responsibility; experience working with individuals with a history of mental illness and substance abuse; knowledge of co-occurring disorders and evidence-based treatment approaches. Proficient in the area of program development and evaluation. Working knowledge state and federal regulations relating to mental health services and substance abuse treatment; including HIPAA and Medi-Cal insurance. Must possess knowledge and skills related to social service case management, supervising program service delivery, and outcome measurements.
Education: 1) Master degree in Social Work, Marriage & Family Therapy, Public Health, or related field; 2) Bachelor degree in social work, psychology, human services, or related field and 2 years of supervisory experience; or 3) High school diploma and 2 years of experience managing social service programs.
License or Certificate: Valid California Class C Driver’s License and clean driving record required (verified by DMV at hire and periodically). First Aid and CPR certifications obtained within 90 days of employment and maintain current thereafter.
Other: Criminal background clearance required.

Interfaces with all levels of management, staff, clients, residents and families including partner agencies.
PHYSICAL SKILLS AND ABILITIES
Ability to perform the essential job functions consistent safely and successfully with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift and carry up to 20 lbs.
Must be able to talk, listen and speak clearly on telephone.
Must be able to conduct outreach to homeless encampments for periods of up to 4 hours.
WORKING CONDITIONS
Hours: Work hours can vary and may require some on-call scheduling including weekends, evenings, and holidays per program requirements.
